Ethical Leadership in Healthcare: A Multilevel Framework for Physician Leaders

Physician leaders face unprecedented ethical challenges navigating healthcare’s transformation — from artificial intelligence implementation to private equity acquisitions, from clinician burnout to organizational pressures prioritizing financial metrics over patient care. Although leadership ethics are critical to organizational culture and patient outcomes, physician-specific ethical leadership frameworks remain scarce. This article proposes a comprehensive, multilevel conceptual framework grounded in organizational behavior and bioethics literature. The framework integrates ethical and unethical leadership behaviors across individual, dyadic, group, and organizational levels, identifying antecedents (personality traits, emotional regulation, organizational culture), observable leadership behaviors (moral signaling, role modeling, decision-making processes), and cascading consequences (clinician well-being, psychological safety, patient outcomes). Unlike traditional models focusing on follower perceptions, this framework emphasizes observable, learnable behaviors that physician leaders can develop. By distinguishing ethical from unethical leadership manifestations and providing evidence-based guidance for navigating complex ethical dilemmas, this framework serves as a foundation for leadership development interventions, assessment tools, and organizational change initiatives aimed at fostering cultures of integrity within healthcare systems.
